The City's Seniors Services and Long-Term Care Division supports Toronto seniors and residents in long-term care homes to have the healthiest, most fulfilling lives possible through exceptional care and services. Vulnerable individuals who reside in the community are supported through adult day programs, supportive housing services, tenancy support and homemakers and nurses services. Please note this posting may be used to fill upcoming vacancies across our 10 Long-Term Care homes as they arise. Develops, evaluates and modifies care plans for residents. Contribute to the development of the person-centred philosophy of care
Conducts various nursing assessments as required and when there is a change in resident/client's health status.
Documents resident conditions and care provisions in the resident/client health care record.
Provides nursing care, including counseling
Assesses the physical, emotional and psychological well being of resident/client through observation and analysis of test and physical health indicators to evaluate care plan and identify modifications which may be required
Accompanies physician on rounds, providing information on resident/client condition and progress, and assisting with examinations, e.g., positioning, dressing and undressing residents/clients
Transcribes verbal or written physician's orders for medications or other treatments
Prepares precise dosages of medication in accordance with physician's orders, including narcotic and controlled drugs, and administers to residents/clients
Maintains inventory, ensuring medications are properly stored, safe and secure
Administers prescribed treatments as ordered by the physician, such as catheterizations, respiratory therapies, suctions, exercise, wound irrigations, and hot pack therapies using electronic/manual equipment for treatments, e.g., Administers first aid and emergency health measures, such as CPR
Practices sterile techniques and other infection control procedures to minimize the transmission of infection
Monitors and participates in the personal care of residents/clients, as required; for example, hygiene, feeding, positioning, transferring, ambulation, toileting
Checks resident/client diet for therapeutic composition
Liaises, refers and advocates with community resources to coordinate admittances, transfers, and departures/deaths: hospitals (acute care, rehabilitation, and palliation), public health department, home care program, long term care facilities, ambulance services, diagnostic services, the coroner's office and social services
Prepares and maintains documentation such as charts, nursing records, statistics, and reports
Performs delegated Medical Acts (duties) i.e. vaccinations, innoculations, eye irrigations, death pronouncement etc
Counsels, instructs, and confers with resident/client and family in planning care: provides emotional support; responds to enquiries and resolves complaints
Provides work direction, coaching and mentorship to BSU staff
Collaborates with the Psychogeriatric Outreach Program (POP) Team, Psychogeriatric Resource Consultant (PRC), Ontario Health at Home, Acute Care, Tertiary Care, other LTC homes, families, and circle of care
Maintains current knowledge of dementias, responsive behaviours, mental health, and relevant diagnoses
Contributes to the BSO community of practice
Assumes responsibility for personal development through participation in ongoing training to enhance skills
Leads and participates in Behavioural Management rounds and care conferences as required
Collaborates effectively when interacting with residents, family, team members and inter- professional care teams
Trains student nurses
Promotes the residents' integration to their environment
Provides information to identify caregiver strategies that are resident-centred, age appropriate, maintain the resident's dignity when responding to the behaviours and managing the associated risks
Documents observational behaviours, recognize that they have meaning, assess and account for how the behaviours impact the care delivery process.
Current registration as a Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO)
- Baccalaureate degree in Nursing or an approved Registered Nursing program equivalent.
- Experience in long-term care environment, mental health, or geriatric populations.
- Experience working in a specialized behavioural program, or populations with responsive behaviours, or completion of behavioural management training.
Current Basic Cardiac Life Support (BCLS) certification
Certification in GPA (Gentle Persuasive Approach), PIECES (Physical, Intellectual, Emotional, Capabilities, Environment and Social), DementAbility, U-First (Understand, Flag, Interact, Reflect and Support), Montessori Methods is an asset
Knowledge of responsive behaviours related to dementia, delirium, mental health, or other relevant conditions
Knowledge of gentle behavioural management techniques and approaches
Ability to coach, engage, and support staff caring for residents with responsive behaviours
Demonstrated ability to lead the care team
Excellent care planning skills
Creative problem-solving skills
Knowledge to monitor staff performance and the environment ensuring a safe workplace for staff; Competent with computer systems, proficient with Microsoft Office applications (Word, Excel, Power Point, etc.), also proficient in using Point Click Care
Ability to develop, coordinate, monitor care delivery and evaluate care plans
