Case Manager - Justice Access & Support System Navigator

The Jean Tweed Centre (JTC), together with the Province of Ontario’s Justice Centres, present an exciting opportunity for an experienced Case Manager-Justice Access & Support System Navigator in Toronto’s first of its kind, community court model, the Toronto Downtown East Justice Centre (DTE-JC).

Jean Tweed Centre is a leading, community-based organization that provides a safe and supportive environment for women with substance use, mental health, and gambling concerns. Recognizing the diversity of women’s experience in today’s society, our programs seek to address a range of issues in a woman’s life that may contribute to substance use and gambling concerns. Understanding each woman’s experience within the broader physical, emotional, social, cultural and gender framework, we provide a client-centered approach to health.

Justice Centres are an innovative court model that integrate early supports into the justice process by bringing together justice, health, employment, education, and social services to address the root causes of crime, break the cycle of offending, and improve public safety and community well-being. Toronto’s Downtown East Justice Centre focuses on breaking the cycle of offending by addressing the root causes of crime for accused at the intersection of poverty, homelessness, mental health and addictions challenges.

The new Case Manager-Justice Access & Support System Navigator will engage exclusively with adult women participants of the Toronto Downtown East Justice Centre, providing support in and outside of their criminal court process. The role involves, but is not restricted to, service navigation, outreach, court accompaniment, short-term counselling, group support, warm referrals, intensive case management services, and internal links in the JTC’s gender-specific continuum of care, to criminal justice involved female participants in the Downtown East Justice Centre. 

Case management activities will include working collaboratively with DTE JC participants using trauma and violence informed approaches to create a needs-based service plan, liaising with mental health and substance use services and service providers, psychiatrist, nurse practitioners and general practitioners, as well as community social services and housing workers.

The role requires active engagement with the DTE JC team which includes MAG Crown counsel and case management coordinator, and other community partners including Regent Park Community Health Centre, LOFT 416 Community Centre for Women, The 519, Surrey Place and Streets to Homes.

This is a unique and exciting opportunity to apply a first of its kind gender-responsive approach to supporting women offenders involved in the criminal justice system. 

Service Responsibilities: 

• Establish a comprehensive, wholistic, individualized service plan with each participant based on their identified needs

• Lead a collaborative wrap-around process with other community-based organizations and service providers to ensure comprehensive service coordination that addresses individual needs. 

• Provide intensive case management support including counselling, outreach, on-going assessment, goal planning, referral, development of support systems, system navigation, etc. 

• Provide linkages to the wrap around services at Jean Tweed such as Nurse Practitioner support, the Child Development Centre, intensive live-in as well day treatment and online programs. 

• Provide linkages to long term supports including mental health and justice permanent housing 

• Actively engage with the DTE JC “team” including the Ministry of the Attorney General as well as other multi-sector community partners including Mental health, Substance Use and Justice system participants 

• Actively assist in the development of gender-responsive, and trauma informed programming for the female participants of the court 

• Complete required case notes, evaluations, and data entry reports in an accurate and timely manner. 

• Monitor the progress of clients and maintain documentation of relevant client interactions and interventions, and contacts with collateral supports and prepare reports and letters as necessary. 

• Participate in staff meetings, team development and supervision meetings. 

• Collaborate with supervisor to discuss job performance and establish professional goals. 

Organizational 

• Conduct job responsibilities with a commitment to harm reduction, trauma- informed, women-centred and anti-oppressive practices. 

• Abide by the Occupational Health and Safety Act, and work in a manner that is safe, reporting incidents immediately to direct supervisor. 

• Adhere to the policies and procedures of The Jean Tweed Centre. 

• Perform other duties as assigned in order to meet the overall goals and objectives of The Jean Tweed Centre. 

• Maintain a client service orientation including learning about and addressing client needs and concerns. 

• Work collaboratively including respect others, sharing information, consulting and celebrating shared successes.

This is a 1-year Contract Role (Union).

Pay:  CA$29.70 - CA$34.75 per hour

Hours of Work (37.5hrs/week):   9am-5:00pm

Location: 35 Britain St, Toronto, ON M5A 1R7

Requirements

What will you bring?

• Bachelor’s Degree in Social Work, or a related discipline. 

• A combination of 3 years of work experience in the field of, Substance Use, Criminal Justice, Mental Health and/or related field. 

• Expertise in providing counselling and case management to individuals who are experiencing or have experienced a complex history of substance use, mental health concerns, and involvement in the criminal justice system. 

• Working knowledge of best practices in mental health, substance use, harm reduction, trauma and the therapeutic relationship. 

• Working knowledge of trauma-informed practice, women centred principles and best practices for women and substance use. 

• Working knowledge of psycho-social rehabilitation and recovery principles. 

• Working knowledge of trauma-informed practice, women centered principles and best practices for women and substance use. 

• Working knowledge of the criminal justice system, housing, health care and community resources in the Toronto area and related provincial resources. 

• Ability to work independently with minimal day-to-day supervision. 

• Crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills.
