St. Joseph’s Care Group is expanding capacity at Hogarth Riverview Manor to better serve our community and strengthen the broader health system.

As part of this exciting expansion, 14 new Community Transitional Care beds will be introduced to support individuals who no longer require hospital-level care but still need additional assistance while transitioning to community living. This initiative creates meaningful opportunities to be part of a team dedicated to providing compassionate, people-centered care at a critical point in each person’s journey.

ABOUT THE ROLE

Under the direct supervision of the Manager Housing & Long Term Care Community Support Programs the Client Counsellor will provide tenant-focused support services and/or comprehensive flexible interventions to clients of Aspen Place.

WHAT YOU BRING

Education/Experience:

• A degree or diploma in one of the social sciences
• Two (2) years experience working with the elderly within a community care environment
• Knowledge and experience with gerontology Alzheimer Disease and related dementias

Skills/Abilities:

• Ability to communicate effectively and concisely both orally and in writing
• Strong facilitation skills along with the ability to deliver presentations information to a variety of audiences
• Ability to establish rapport with others and to maintain effective working relationships with superiors co-workers and customers
• Strong time management and organizational skills
• Ability to identify understand and meet the requirements of internal and external customers- Ability to work independently and as part of a team
• Demonstrated regular attendance at work

Asset:

• Must maintain membership in the Ontario College of Social Workers and Social Service Workers

Conditions of Employment:

• This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
