Location: Lakeshore Care Centre – Coquitlam, BC

About Lakeshore

Lakeshore offers 56 licensed residential care beds; 35 Fraser Health subsidized care beds and 21 private pay.  Care beds are primarily single occupancy with limited double occupancy available.  Our rooms are both private and semi-private.
Our home is conveniently one story and has large outdoor patios where many activities are held throughout the spring and summer months.
Lakeshore Care Center works in collaboration with the Fraser Health Authority.

 

What You’ll Do
As an LPN, you’ll play a vital role in providing resident-centered care . Your responsibilities will include:

• Supporting resident admissions, discharges, and transfers
• Completing assessments and contributing to individualized care planning
• Administering medications, treatments, and wound care
• Documenting care in PointClickCare and completing MDS assessments
• Liaising with physicians, families , and interdisciplinary teams
• Delegating tasks to care aides and participating in care conferences and medication reviews

What You Bring

• Active registration as an LPN in good standing with BCCNM
• Current CPR and First Aid certification
• Proficiency in electronic documentation systems (e.g., PointClickCare, eMAR)
• Strong communication skills in English (both verbal and written)
• Effective critical thinking and decision-making skills
• Previous experience in long-term care (preferred)
