Mental Health Case Manager in Thunder Bay

Join Dilico Anishinabek Family Care as an Adult Mental Health Case Manager in Thunder Bay, Ontario. This vital in-city role involves implementing an effective case management system for high-risk adults. You'll engage in assessments, counselling, and multidisciplinary care coordination to meet the diverse needs of clients. Make a real impact in the community by delivering integrated services from intake to discharge while maintaining respectful relationships with Anishinabek individuals and families. Key Responsibilities:
• Ensure documentation and reviews for clinical files
• Evaluate and recommend changes to the case management system
• Liaise with inpatient and community mental health services
• Provide assessments and counselling consistently
• Maintain cooperative work relationships with clients and services Requirements:
• HBSW or related degree; MSW preferred
• Three years of case management experience
• Knowledge of Anishinabek culture and language
• Valid Class “G” Driver’s License and personal vehicle
• Standard First Aid/CPR Certificate Make a difference in mental health management with Dilico in Thunder Bay.
