Job Summary

Provide comprehensive primary care to patients of all ages delivering evidence-based diagnosis treatment and preventive services. Coordinate patient care with specialists and allied health professionals while maintaining accurate clinical documentation.

Main Duties

• Obtain detailed medical histories perform physical examinations and order or interpret diagnostic tests to establish diagnoses.
• Develop implement and adjust individualized treatment and care plans including prescribing medications and therapies.
• Deliver preventive care services such as routine screening immunizations health promotion and lifestyle counselling.
• Manage acute illnesses and exacerbations of chronic conditions and perform minor clinical procedures as appropriate.
• Coordinate referrals to specialists collaborate with multidisciplinary teams and follow up on patient progress and specialist recommendations.
• Maintain accurate and timely medical records using electronic medical record systems and ensure appropriate follow-up and continuity of care.

Requirements

• Doctor of Medicine (MD) degree or equivalent from an accredited institution.
• Completion of a residency program in family medicine or equivalent postgraduate training.
• Certification or eligibility for certification by the College of Family Physicians of Canada or an equivalent certifying body.
• Eligibility for registration and licensure with the provincial or territorial medical regulatory authority.
• Clinical experience in primary care or family medicine demonstrating competence in diagnosis and management.
• Proficiency in English with effective verbal and written communication skills; ability to communicate in French is an asset.
