RAI & Restorative Lead Coordinator Full Time

Reporting to the Director of Care the MDS-RAI Coder is responsible for review of interdisciplinary and comprehensive resident assessments. Collection of data for the purposes of quality documentation management and Resident Assessment adherence to the Minimum Data Set (MDS) and InterRAI LTCF requirements. providing support to the nursing and accounting departments.

RESPONSIBILITIES

• Responsible for managing the co-ordination and completion of the Resident Assessment Clinical Assessment Protocols and care plans as assigned.
• Complies with MLTC regulations regarding completion of the InterRAI LTCF process.
• Completes accurate coding of the InterRAI and care plan documentation for all residents.
• Continually updating knowledge base related to data entry and computer technology.
• Chair RAP chat as required.

• To provide restorative education support and assistance to staff and residents;

  To assess each new resident in regard to restorative needs;

  To develop and document restorative goals and individualized plan of care for resident;

  To re-evaluate care plan as per restorative care policy and procedure

  ; To monitor and train staff regarding restorative tasks and resident specific goals;

  To record statistics and keeps records as required activities;

  To coordinate Bed Entrapment Prevention Program;

  To use supplies and equipment as directed and provide input in equipment selection;

  To attend care conferences as required and participates in Quality Management;

  To participate in the quality management program and complete restorative audits as required;

  Chairs restorative committee;

  To participate in setting realistic and achievable departmental goals;

  To complete and ensure residents care assessments service plans and documentation are up to date and changes communicated to DOC for billings;

  To assist with scheduling of residents care conferences and participate in conferences;

  To facilitate and provide resident/family education;

  To coordinate nursing teams resident care assignments to ensure efficient and timely delivery of services to residents;

  To respond to and manage significant changes in residents condition and or medical emergencies;

  To respond to resident/family concerns and ensure appropriate action taken and or concern communicated to DOC/Executive Director and other departments;

  To assist the DOC in the coordination implementation and monitoring of Infection Prevention and Control Health and Safety and Clinical Programs and Quality Plans for the Nursing Department;

QUALIFICATIONS

• 3-5 years experience in Long Term Care experience with MDS an asset.
• Current registration as RPN in good standing with the College of Nurses is required.
• Current InterRAI certification and Relias certification is required.
• Must be able to maintain complete confidentiality of information.
• Knowledge and experience with adult education strategies/facilitation.
• Strong analytical skills; knowledge in trending and analysing data.
• Well-developed oral and written (legible) communication skills.
• Proficient computer skills and strong problem-solving abilities with excellent time management.
• Clear aptitude for detail-oriented work.
• Strong understanding of Point Click Care and RAI-MDS.
• Excellent organizational communication and interpersonal skills
• Must be flexible to work variable shifts self-directed and able to function in a fast-paced environment

Candidates will be required to provide a Vulnerable Persons Sector Police Check (acceptable if within 6 months).

Hours of work for this position require flexibility including days evenings and nights as well as statutory holidays and weekend shifts as needed.
