Life Redesign Coach (Care & Community Worker)

As a Life Redesign Coach , you provide direct day-to-day support to adults living with brain injury and stroke at CONNECT. This is a hands-on care and community support role delivered within a coaching framework. We promote a "do with" approach instead of "do for".

You work as part of a multidisciplinary team that includes allied health professionals and support staff. Your role is to support individuals to live safely, meaningfully, and as independently as possible within a shared living environment.

This work includes both personal care and community-based support , such as:

• Assisting with personal care (bathing, dressing, toileting, hygiene, mobility, and continence support)
• Preparing meals and supporting nutrition, including cooking and feeding assistance when required
• Supporting medication delivery and routine health-related tasks as delegated
• Driving or accompanying individuals to appointments, activities, and community access
• Supporting exercise, rehabilitation, and daily living routines as part of goal-based care
• Responding to day-to-day needs in a shared home environment
• Supporting participation in recreation, social activities, and community life

This is a practical, relational role. Staff are expected to be actively involved in supporting residents throughout the day, not only supervising or prompting.

No formal care aide certification is required at entry. Full training and orientation are provided. We hire for attitude, reliability, emotional tolerance, and alignment with our care approach.

• Supporting people with brain injury and stroke in real-life environments
• Building independence through daily practice, not institutional routines
• Working alongside individuals (“doing with”), while still providing necessary hands-on support
• Encouraging participation, safety, and dignity in all aspects of care
• Balancing autonomy with appropriate levels of support and supervision

QUALITIES REQUIRED

This role requires:

• Emotional tolerance and calmness in complex care situations
• Respect, patience, and consistency in daily support work
• Willingness to provide personal and sometimes physically demanding care
• Ability to work in a shared home environment with competing needs
• Strong teamwork and communication
• A grounded, practical approach to supporting people with disability

Experience in brain injury, disability support, or caregiving is an asset, but not required.

REQUIREMENTS

• Criminal Record Check (Vulnerable Sector)
• First Aid and CPR
• Food Safe certification
• TB screening (current)
• Immunization declaration
• Valid driver’s licence and clean abstract
