Peterborough Family Health Team (PFHT) is a collaborative primary care organization that works alongside family physicians and community partners to provide comprehensive, team-based care to the residents of Peterborough and Peterborough County. Our interdisciplinary team works together across multiple clinical sites.

PFHT supports clinical excellence, innovation, and digital health solutions that enhance patient care, improve provider workflows, and protect the privacy and security of health information.

We are currently seeking a 1.0 FTE full-time contract from time of hire to May 14, 2027, to support the expanding needs of the organization and to ensure consistency in service delivery.

Overview

The Registered Practical Nurse (RPN) reports to the Patient Services Supervisor at the Peterborough Family Health Team (PFHT) and is an integral member of the Connect Clinic (unattached patient clinic) team based in Peterborough, Ontario.

Working within an interprofessional, multidisciplinary team that includes fee-for-service physicians, Nurse Practitioners, Registered Practical Nurses, and Medical Office Assistants, the RPN provides episodic primary care to individuals who do not have a family physician or nurse practitioner. Practicing within their full scope as defined by the College of Nurses of Ontario (CNO) Standards of Practice, the RPN conducts comprehensive health histories and clinical assessments, delivers nursing interventions, supports care coordination, and facilitates timely navigation into appropriate primary care and related services. The role supports the ongoing clinical operations of the Connect Clinic, building on established workflows to promote equitable, high-quality, and person-centered access to care.

Responsibilities

Registered Practical Nurse (RPN) will provide episodic primary care and nursing support to unattached patients within the Connect Clinic, practicing within the College of Nurses of Ontario (CNO) Standards of Practice. Key responsibilities include:

• Conduct patient interviews to obtain and document comprehensive health histories, presenting concerns, medications, allergies, and intolerances in the EMR
• Perform focused and comprehensive health assessments using appropriate clinical tools, incorporating physical, psychosocial, cultural, and spiritual considerations
• Apply clinical reasoning and evidence-based guidelines to identify health concerns, support screening, and contribute to care planning
• Review and maintain accurate medication lists, assess adherence, and provide medication education
• Deliver nursing interventions including immunizations, wound care, injections, vital signs monitoring, and assistance with minor procedures
• Provide in-person and telephone-based triage, follow-up, and patient support in accordance with established protocols
• Incorporate laboratory results, diagnostic tests, and specialist reports into ongoing assessment and care coordination
• Support chronic disease management, preventive health activities (including flu clinics), and patient self-management education
• Collaborate closely with physicians, nurse practitioners, and medical office assistants to coordinate care, follow up on test results and referrals, and support care transitions
• Maintain accurate, timely, and professional clinical documentation in alignment with organizational, professional, and legal standards
• Uphold infection prevention and control practices and contribute to clinic efficiency, data collection, and quality improvement initiatives
• Carry out other related duties as required to support the effective operation of the Connect Clinic and PFHT programs and services

Position Requirements

• Ability to perform all clinical and technical tasks within the RPN scope of practice
• Strong interpersonal and communication skills with sound clinical judgment
• Ability to work independently and collaboratively within a busy, multidisciplinary environment
• Proficient in the use of Primary Care Electronic Medical Records (e.g. Telus PS Suite, Accuro) and other relevant digital tools(e.g. Ocean, Connecting Ontario)
• Understanding of how social determinants of health impact individual and community health
• Demonstrated commitment to anti-racism, anti-oppression, equity, and inclusion principles
• Demonstrated problem-solving skills and adaptability in a fast-paced environment
• Familiarity with primary care and how it intersects with other parts of the healthcare system
• Willingness to work in full scope of practice, performing delegated acts through medical directives
• A valid driver’s license and reliable vehicle (travel to various clinic locations may be required)
• Clear vulnerable sector police check
• Willingness to participate in data collection for quality improvement and program development
• Ability to lift up to 20 lbs
• Flexibility to adapt to changing hours and clinical demands

Hours Of Work

Hours of service are pre-established per employment agreement and may require some evenings and weekends. Any deviation from the established schedule shall not negatively impact upon the provision of service to the patient population.

Qualifications

• Diploma in Practical Nursing
• Minimum 3 years’ clinical experience, preferred primary care
• Registered in good standing with the College of Nurses of Ontario (CNO)
• Active knowledge of CNO Standards and Guidelines
• Excellent documentation and time management skills
• Experience in immunization administration
• Well-women’s and well-baby assessment skills an asset
• Member of WeRPN preferred
