As a Nurse Next Door Care Designer / Care Manager you enjoy working collaboratively but with a high degree of autonomy and you thrive by making client care come to life and seeing results. In addition to understanding the importance of client care, in this role you also understand the importance of clear, actionable plans that deliver results.

As a Care Designer, a day in the life will require:

• Comfortable in meeting families to design client centered care
• Taking a solutions oriented approach, saying yes to clients
• Flexibility & having a sense of urgency when following up with client needs
• Self-starter with the ability to have intentional, compassionate conversations
• A bold passion for delivering client centered care in home and community settings
• A business mind and customer service background, health care experience is an asset
• Leading a team of Caregivers to provide clear goals, tasks, coaching and education
• Promoting a culture of client centered care, emphasizing premium service and compassion
• Coordinating care, implementing clinical solutions, and evaluating client outcomes based on client goals
• Collaborating with the Care Coordinator to achieve scheduling goals, respond to urgent inquiries, scheduling as needed
• Bring a continuous improvement mindset to the role and the team, always look for ways to improve the experience of our clients, caregivers and referrals
• Participate in the after-hours on-call rotation with other Care Designers. We are a 24/7 caring business.
• Respectful, flexible, likes seniors and concerned about their care

Education and Experience Requirements: Customer service and management background Three to five years' experience in a home care environment or senior care*

• RPN preferred
• Car, valid driver's license and car insurance

Pay: $65,000.00-$70,000.00 per year
