Program Summary:
The Path Home - Champagne is a specialized Alternate Level of Care (ALC) Transitional Care and Inpatient Community Program operated through a partnership between LOFT Community Services and the Centre for Addiction and Mental Health (CAMH). The program supports individuals transitioning from hospital care to community living by providing enhanced clinical, personal, and housing-focused supports that foster stability, independence, and successful reintegration into the community. Clients receive 24/7 nursing and personal care, case management, care coordination, and life-enrichment services delivered by an interdisciplinary team. In partnership with CAMH primary care and psychiatric physicians, the program provides recovery-oriented, person-centered care that supports wellness, dignity, and transitional housing and community integration.

Job Summary:
As the Registered Practical Nurse , you will be part of an interdisciplinary team, who is expected to ensure the safety and security of the resident environment, to practice in accordance with College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services. The RPN will be responsible for working with individuals from diverse backgrounds, contributing to Program development, participating in clinical supervision; and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. The RPN is directly responsible for developing and articulating strategies for the management of recovery and psychosocial rehabilitation using information collected through assessments and collaboration with the PSWs. Additionally, you will liaise with community agencies, other practitioners, and key partners to coordinate care for clients.

What You Will Do:

• Provide direct nursing care and medication management, including safe medication administration, order transcription, medication ordering and documentation (MAR), secure medication storage, and compliance with College of Nurses standards, pharmacy protocols, and organizational policies.
• Monitor and assess clients’ physical, mental, and behavioural health, reporting changes, concerns, and incidents as appropriate.
• Support client care planning, goal setting, and successful transitions to long-term or discharge destinations.
• Respond to behavioural, cognitive, and crisis situations, including conflict resolution and emergency intervention as needed.
• Conduct assessments, maintain accurate documentation, and ensure required supplies and equipment are available.
• Promote a safe, clean, and supportive environment while following infection prevention and control practices.
• Collaborate with physicians, pharmacists, healthcare professionals, community partners, clients, and families to coordinate quality care.
• Provide clinical guidance and support to staff on health-related matters.
• Foster respectful, recovery-oriented relationships with clients while maintaining confidentiality and professional standards.
• Participate in on-call rotations, team meetings, training, care conferences, and other organizational activities.
• Support interdisciplinary team operations and perform additional duties as assigned, including backup support for Personal Support Workers.
• Perform other related duties as assigned in support of program operations and client care.

What You Bring:

• Completion of a recognized RPN program with medication administration training/certification.
• Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN).
• Experience supporting older adults and seniors with mental health, cognitive, addiction, and responsive behavioural challenges.
• Valid First Aid and CPR certification, with a clear Criminal Record and Vulnerable Sector Check.
• Knowledge of community resources, social services, and supports available to adults and seniors.
• Strong understanding of recovery-oriented care, psychosocial rehabilitation, and therapeutic relationship principles.
• Excellent assessment, problem-solving, counselling, communication, and interpersonal skills.
• Ability to work independently and collaboratively within an interdisciplinary team environment.
• Proficient with MS Office applications and clinical documentation.
• Physically capable of performing client care activities, including lifting, transferring, and operating mobility equipment.

Work Environment:

• Occasional evening and weekend hours may be required.
• Physically able to perform demanding body mechanics i.e. lifting, transferring, Hoyer, etc.)
