Description

The Hospital to Home (H2H) program is a short-term care initiative designed to help patients transition from hospital to home, particularly when they no longer require acute care but need extra support. The goal is to support timely discharge from hospital to reduce awaiting long-term care (ALC) and reduce 30-day readmission rates.

The Hospital to Home (H2H) Clinical Information Clerk is responsible for reception, clerical, and office administrative duties for the Hospital to Home Program.

This position provides administrative and clerical support to ensure the efficient operation of the Hospital to Home Program. Responsibilities include scheduling appointments, maintaining accurate patient information and documentation in the EMR, preparing and processing intake information, managing supplies, recording meeting minutes, processing discharged files, and completing other administrative duties as required.

Qualifications

Education

• Grade 12 or equivalent
• Must have Medical Terminology Certificate, or be willing to complete training

Experience

• Previous experience in a clerical/administrative role that includes reception and office procedures, applicants with other combinations of applicable experience and education may be considered

Knowledge & Skills

• Demonstrated positive interpersonal skills and ability to communicate effectively
• Ability to work in a team environment
• Ability to organize and prioritize work with time constraints
• Reliable attendance record
• Knowledge of CPI and recertification an asset
• Proficient in the use of computers and other hardware including photocopiers and printers
• Proficient with MS Office Suite
• Excellent keyboarding skills; minimum 35 WPM (subject to testing)
• Must participate in required cultural competence training and demonstrate appropriate cultural competence
