Job Responsibility:

Job Title: Nurse Practitioner (NP)

Location: Oakville

Type: Full Time, Permanent

Rate: $110-125k/annum

Timings: Monday - Friday (Morning)

Requirements:

· Registered Nurse Practitioner in good standing with the College of Nurses of Ontario in the extended class.

· Valid CPR certification and VSC.

· Minimum of three years of community health care and/or primary care experience.

· Experience in addiction medicine and harm reduction.

· Excellent clinical skills and thorough current nursing knowledge.

· Proficiency in conducting comprehensive health histories, physical assessments, and constructing nursing management plans.

· Valid driver's license and access to a reliable vehicle. (Must have)

· Successful completion of the Prescribing Narcotics and Controlled Substances course is a major asset.

· Completion of a Suboxone online education program is a major asset.

· Certifications in non-violent crisis intervention, First Aid, CPR, and ASIST are a plus.

· Experience with LOCUS, OCAN, and VI-SPDAT assessments is a plus.

· Membership in specialty-related organizations (e.g., NPAO, RNAO) is a plus.

Responsibilities:

· Deliver primary care services within NP scope of practice, including health assessments, physical exams, therapeutic management, and referrals for substance users.

· Assess, diagnose, and treat patients using critical thinking and evidence-based practices.

· Provide holistic patient care based on advanced nursing and medical management knowledge.

· Develop and monitor care plans with clients and their circle of care.

· Adjust medication and treatment based on client responses, including withdrawal management.

· Consult with physicians for clients with complex conditions.

· Perform physical exams, order tests, and prescribe medications for substance use disorders.

· Write prescriptions and order lab tests within NP scope of practice.

· Provide supportive counseling and short-term crisis management.

· Conduct home visits to clients at community centers, shelters etc. within the Halton Region.

· Ensure quality of care meets College of Nurses standards, Regulated Health Professions Act, and SH policies.

· Utilize health promotion approaches to address social determinants of health.

Pay: $110,000.00-$125,000.00 per year
