As a Clinical Pharmacist, you will provide essential patient-focused pharmaceutical care, ensuring medication safety and effectiveness across various departments including medicine, surgery, and critical care. This role involves electronic verification of medication orders, patient education, and mentoring pharmacy students. Your expertise will drive the development of pharmacy standards and practices while supporting drug distribution activities. Key Responsibilities:
- Provide direct patient care and medication management
- Support drug distribution and supervision of pharmacy assistants
- Conduct educational programs for staff and students
- Assist with the development of pharmacy protocols and guidelines
- Facilitate intravenous admixture services and medication reconciliation
- Doctor of Pharmacy or Bachelor of Pharmacy degree
- Registration as a Part A Pharmacist in Ontario
- Experience in clinical hospital pharmacy preferred
- Strong communication and teaching skills
- Ability to supervise and coordinate pharmacy operations
