Nurse Case Manager

Cencora seeks a licensed nurse case manager to support patient enrollment, therapy coordination, and access to prescribed treatment through clinical and operational procedures. The role involves reviewing patient files, auditing case activity, evaluating access barriers, and coordinating across healthcare providers and internal teams.

Responsibilities

• Review patient files, treatment history, and documentation to confirm readiness for case action
• Execute clinical and operational procedures to support enrollment, therapy coordination, and treatment access
• Audit case activity, follow-up actions, and documentation quality for discrepancies and process gaps
• Evaluate therapy access barriers and coverage issues to determine escalation and resolution needs
• Coordinate with healthcare providers, pharmacies, and clinics to stabilize patient therapy access

Requirements

• Licensed Nurse (RN or RPN/LPN) with active provincial registration
• Diploma in healthcare or related field; Bachelor's degree preferred
• Knowledge of clinical operations, care coordination, and case management
• Knowledge of patient support programs, therapy access pathways, and reimbursement processes
• Ability to interpret clinical and operational information accurately
• Ability to coordinate across patients, providers, pharmacies, and internal teams
• Knowledge of adverse event reporting, quality controls, and compliance standards
