Expected Start Date: June 15, 2026 Union: HSAS Facility: Broadway Medical Clinic City/Town: Yorkton Department: Chronic Disease Management Collaborative Type: Full-time regular FTE: 1 Shift Information: Days Number of Hours per Rotation: 14 shifts of 8 hours per 3 weeks Relief: No Float: No Hours of Work: Standard Hours Salary or Pay Band: Pay Band Mental Health Therapist - Staff $35.546 to $43.466 Travel Required: Yes Job : The Primary Health Care (PHC) Counselor reports to the Primary Health Care Manager-Central. Professional peer review and responsibility will be maintained through a joint accountability to Mental Health/Addictions. The incumbent is a member of the PHC team, in a collaborative, co-location model. The incumbent is responsible for ongoing mental health and psychosocial services for their clients. This position screens and assesses the needs of referred clients and determines a treatment plan which may include short term intervention with the counselor and/or case management or referral to other community programs. The PHC Counselor initiates/participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. Incumbent promotes quality health care outcomes working within established standards of practice ensuring clients receive a high standard of primary health care. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement" Human Resources Exemption: No Education- Bachelor of Science in Psychology
