Program Summary:
Since 1984, The 416 Community Support for Women (The 416) has provided a safe and supportive environment for women (cis and transgender) who are experiencing or are at-risk of experiencing homelessness, and/or are facing mental health and substance use challenges. Our team works primarily with women living with unmet needs due to their complex challenges.

LOFT Community Services, 416 program together with the Province of Ontario’s Justice Centres, present an exciting opportunity for an experienced Case Manager in Toronto’s first of its kind, community court model, the Toronto Downtown East Justice Centre (DTE-JC).

Justice Centres are an innovative court model that integrate early supports into the justice process by bringing together justice, health, employment, education, and social services to address the root causes of crime, break the cycle of offending, and improve public safety and community well-being. The Toronto Downtown East Justice Centre focuses on breaking the cycle of offending by addressing the root causes of crime for accused at the intersection of poverty, homelessness, mental health and addictions challenges. 

Job Summary:
As a Case Manager you will work independently and be part of a team that co-ordinates a support network and links clients living with mental illnesses, substance use and physical health issues. The position attempts to empower participants, thereby increasing their independence, dignity and self-respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.

The Case Manager will engage exclusively with adult women participants of the Toronto Downtown East Justice Centre (DTE-JC), providing support in and outside of their criminal court process. The role involves, but is not restricted to, service navigation, outreach, court accompaniment, short-term counselling, group support, warm referrals, intensive case management services, and internal links to LOFT’s available supports, to criminal justice involved women participants of the Toronto Downtown East Justice Centre. Case management activities will include working collaboratively with DTE JC participants using trauma and violence informed approaches to create a needs-based service plan, liaising with mental health and substance use services and service providers, psychiatrist, nurse practitioners and general practitioners, as well as community social services and housing workers.

The role requires active engagement with the DTE JC team which includes Ministry of the Attorney General Crown counsel and Case Management Coordinator, and other community partners including Jean Tweed, The 519, Regent Park Community Health Centre, Surrey Place and Streets to Homes. This is a unique and exciting opportunity to apply a first of its kind gender-responsive approach to supporting women offenders involved in the criminal justice system.

The Case Manager will support their clients while they are in the DTE JC, help them maintain release conditions and work with the participant to achieve individual and community wellbeing goals. LOFT utilizes a Case Management approach in service delivery, which encompasses a continuum of flexible comprehensive interventions to help a client achieve a stable life. This approach emphasizes outreach, assessment, planning, linkages, and service delivery on both an individual and group basis.

What You Will Do:

• Develop respectful and trusting relationships with clients using a trauma informed practice and Psychosocial Rehabilitation principles
• Assist clients in creating individualized plans tailored to their needs and designed to improve quality of life and reflect accountability and recovery
• Support clients in identifying and referring to resources specific to their needs based on assessment
• Coordinate care and act as a liaison by connecting clients with social, medical, and community support.
• Provide crisis intervention and help develop crisis plans.
• Represent the program professionally with clients, families, and community partners.
• Mediate conflicts and help clients learn problem-solving skills.
• Organize or support social activities to reduce isolation.
• Maintain accurate client records and documentation.
• Work collaboratively with the team, contribute ideas, and support new staff or volunteers.
• Provide case management and connect clients with social, health and community services like housing and ID support.
• Plan and facilitate group activities/ events to promote engagement/ participation, development of social networks and to help build skills
• Help clients access financial supports such as Ontario Works (OW) and Ontario Disability Support Program (ODSP) or other financial resources as required.
• Other duties as assigned by the DTE JC Crown Lead in collaboration with the Program Manager and/or designated supervisor.

What You Bring:

• Post-secondary education in Social Services / Human Services or equivalent field.
• Minimum 3 years’ working experience with client population group.
• Understanding of trauma informed practices and an understanding of challenges faced by women who may experience challenges that could include: mental illness, substance use, physical health concerns, and homelessness etc.
• Proficient in intake assessment, crisis intervention, advocacy, and problem-solving to support client needs.
• Ability to build therapeutic relationships and collaborate with clients on goals.
• Familiarity with community resources and professional services.
• Strong teamwork skills in a multidisciplinary environment.
• Strong interpersonal, communication, and counselling skills to effectively engage clients in sensitive areas such as life skills, healthy living, and relationships.
• Solid computer literacy for maintaining records, files, and reports.
• Effective analytical, organizational, and time management skills for coordinating client support and program activities.
• Demonstrates cultural competency, with an understanding of diverse backgrounds and inclusive practices.
• Must have a valid driver’s license, insurance, and a reliable vehicle to travel within the region.

Work Environment:

• Staff will be required to work in clients’ rooms and may therefore be exposed to secondhand smoke.
• May also be required to be on-call as emergency contact for after hours.
• Frequent travel within the Central LHIN will also be required.
• Occasional evening and weekend hours may be required.
