Rehabilitation Service Specialist at Lifemark

Lifemark Health Group

Transform lives as a Rehabilitation Service Specialist with Lifemark, working from home in the Atlantic Region. Deliver vital rehabilitation services to Canadian Armed Forces veterans and their families. In this permanent, full-time role, you will provide comprehensive rehabilitation assessments and develop individualized plans for program participants. Your expertise in managing effective communication and building trust will play a critical role in empowering veterans to improve their well-being. Collaborate with various health professionals to ensure services meet program mandates and address participants' unique needs. Key Responsibilities:

Conduct assessments to clarify rehabilitation needs
Develop thorough rehabilitation plans in consultation with stakeholders
Integrate services across medical, psycho-social, and vocational disciplines
Identify barriers and implement strategies for optimal outcomes
Empower veterans through community resources and interventions

Requirements:

Bachelor's degree in relevant field
Minimum 3 years of rehabilitation service experience
Professional designation in social or health services
Ability to obtain federal security clearance
Reliable vehicle for travel
