Join CarePartners as a Wound Care Nurse specializing in wound, ostomy, and continence care. This role offers flexibility in scheduling and the opportunity to positively impact patients' lives in the St. Catharines and Niagara area. CarePartners seeks a Nurse Specialized in Wound, Ostomy, and Continence (NSWOC) to provide expert consultation in community and clinic settings. You will leverage your skills to promote self-management and lead an interdisciplinary team for optimal outcomes. Ongoing clinical support, a comprehensive orientation, and an iPad for assessments will be provided to ensure your success. Key Responsibilities:
- Conduct assessments and manage complex wound and ostomy care
- Provide consultations for patients and healthcare teams
- Educate patients on wound management techniques
- Collaborate on treatment goals with clients and caregivers
- Monitor healing progress and adjust care plans
- Registered Nurse (RN) with NSWOC certification
- Minimum several years of wound care experience
- Strong interpersonal and communication skills
- Valid driver’s license with a vehicle for travel
- CPR certification and clear background check
