Engagement Case Manager at Salvation Army

Join The Salvation Army as a Direct Engagement Case Manager, serving high acuity shelter clients in need. Utilize your social service experience to drive successful engagement for Housing First initiatives. This role focuses on direct services for clients in emergency shelter settings who require motivation and comprehensive support. You will manage a caseload while employing structured case management methods to facilitate clients' transitions to supportive housing. Your contributions are vital in addressing homelessness through thoughtful assessment and intervention strategies. Key Responsibilities:

Provide direct engagement services to high acuity clients
Assess clients' needs for various social services
Develop and implement service/goal plans with clients
Monitor progress and advocate on behalf of clients
Maintain accurate documentation and reporting to stakeholders

Requirements:

Relevant diploma or degree in social services
Minimum two years experience with homeless populations
Effective communication skills for assessment and reporting
Valid “G” Driver’s License and clear driving record
Bilingualism preferred for client interactions
