Integrated Primary Care Director at SRCHC

Join the South Riverdale Community Health Centre as the Integrated Primary Care Director. Drive transformational primary health care services focused on community needs and health equity. Reporting to the Chief Executive Officer, you will lead the enhancement of integrated primary care programs while fostering collaborative partnerships. This role requires innovative thinking and the ability to lead diverse teams towards effective service delivery and excellence in patient care. Your commitment to equity-driven practice will guide operational initiatives. Key Responsibilities:
- Manage delivery and continuous improvement of primary care programs
- Foster partnerships and community engagement in service initiatives
- Support quality standards and accountability measures
- Lead a culture of excellence within interdisciplinary teams
- Guide strategic planning aligned with health system needs
- Master's degree or significant leadership experience
- Minimum 5 years leading health programs or teams
- Knowledge of community health and primary care models
- Commitment to working with diverse, marginalized populations
- Strong interpersonal and analytical skills
