- Create and maintain community-focused Care Plans
- Deliver nursing services in diverse environments
- Collaborate with patients and families effectively
- Engage with the interdisciplinary care team
- Conduct required clinical duties as needed
- Degree/diploma for RN or RPN
- Good standing with CNO
- Valid driver’s license for travel
- Excellent English communication skills
- Current CPR Certification and immunizations
