La personne retenue favorisera et renforcera l’autonomie des personnes recevant leur congé de l’hôpital en leur offrant des services en fonction de leurs besoins particuliers durant la transition entre l’hôpital et le domicile. Elle collaborera avec les personnes proches aidantes, la ou le responsable de la coordination du programme, les équipes de l’hôpital, les membres de leur famille et les autres prestataires de soins afin d’atténuer les risques susceptibles de mener à des réadmissions. Elle planifiera également la prestation de services sécuritaires qui répondent aux besoins des gens et tiennent compte de leurs préférences.
Tâches et responsabilités
Offrir du soutien aux personnes participantes durant la transition de l’hôpital à leur domicile, conformément au plan de services établi.
Faire de petites commissions, ou en organiser la livraison, afin de favoriser le rétablissement à domicile des personnes en répondant à leurs besoins physiques (alimentation, médicaments, fournitures médicales, équipement, etc.), conformément au plan de service établi.
Formuler des recommandations et offrir des références accompagnées et du soutien aux personnes recevant des services pour répondre à leurs préoccupations.
Travailler dans un esprit de collaboration avec les autres partenaires du cercle de soins des personnes recevant les services afin de respecter les exigences en matière de confidentialité tout en atteignant les objectifs en matière de soins.
Comprendre et effectuer les activités instrumentales de la vie quotidienne de la personne recevant des services, ou accompagner celle-ci dans leur exécution, notamment en facilitant l’accès aux rendez-vous de suivi ou l’atteinte des objectifs de rétablissement.
Faire en sorte que les personnes recevant les services participent à l’établissement d’objectifs et à la mise en place d’activités et de réseaux sociaux permettant d’atténuer les sentiments d’isolement et de solitude.
Repérer toute situation dangereuse ou tout équipement dangereux et les signaler.
Effectuer des évaluations des risques afin de déterminer les besoins en matière de soins des personnes recevant des services et de leur offrir du soutien; par ailleurs, mettre en œuvre des stratégies afin de réduire les risques liés à la santé et à la sécurité.
Remplir tous les documents requis et tenir à jour les dossiers clients normalisés.
Profil recherché
Détenir au moins trois ans d’expérience et un certificat ou un diplôme d’études postsecondaires obtenu au terme d’un programme d’un ou de deux ans en gériatrie, en travail social ou dans un domaine connexe, ou posséder une combinaison équivalente d’études et d’expérience.
Maîtriser le français et l’anglais.
Posséder un permis de conduire valide sans restriction de classe G (ou l’équivalent provincial) ainsi qu’un dossier de conduite satisfaisant.
Être âgé(e) d’au moins 25 ans afin de pouvoir conduire les véhicules de la CRC.
Maîtriser les applications de la suite Microsoft Office (Word, Outlook, Excel, PowerPoint) et SharePoint.
Pouvoir cerner et résoudre les problèmes en appliquant les procédures normalisées et des connaissances fonctionnelles ou organisationnelles.
Être capable de communiquer efficacement, de faire preuve de diplomatie et d’empathie pour interpréter l’information et offrir des conseils, de bien définir les besoins des parties prenantes, de proposer des solutions et de résoudre des enjeux complexes dans des situations nouvelles ou délicates.
Pouvoir réfléchir rapidement et agir en situation d’urgence avec confiance et clarté quant aux procédures normalisées.
Être capable de faire preuve de compassion, d’empathie et de compréhension à l’égard des personnes présentant différentes capacités cognitives et physiques.

Il exige de s’adapter aux changements dans l’ordre des priorités et de répondre aux besoins urgents.
La personne retenue travaillera dans un environnement de bureau, dans des hôpitaux ou au domicile des personnes recevant les services.
L’emploi prévoit de se rendre aux domiciles des personnes recevant les services, et pourrait exiger d’évoluer dans des conditions modérément difficiles où l’on doit user de prudence.
L’emploi exige de travailler de manière autonome aux domiciles de personnes recevant les services et dans des conditions potentiellement imprévisibles; comme la personne pourrait notamment être exposée à des environnements peu sécuritaires ou insalubres, à des allergènes et à de fortes odeurs, une bonne connaissance de la situation et le respect des protocoles de sécurité sont essentiels pour garantir son bien-être personnel et la prestation des soins.
Le travail exige le respect de multiples échéances conflictuelles.
Le travail nécessite des interactions dans des situations difficiles et chargées émotivement.
Il implique d’interagir avec des personnes qui vivent une situation traumatisante et de leur apporter du soutien.
