Are you passionate about helping people regain their independence and improve their quality of life? PHARA Community Services is currently hiring a full-time Rehabilitation Support Worker for its Post Stroke Transitional Care Program.
In this rewarding role, you'll work alongside an interdisciplinary team to support clients through physical and cognitive rehabilitation, life skills training, and exercise programs as they recover from stroke. This is an opportunity to make a meaningful difference in the lives of people across North Bay and surrounding communities.
- Permanent Full-Time; $23.87 - $25.02/hour

KEY RESPONSIBILITIES
The RSW assists/teaches clients in their rehabilitative activities including life skills so that maximum independence and identified goals are achieved. The RSW will lead individual physical and cognitive rehabilitation sessions as well as facilitate exercise groups under the authorization of an OT/PT authorized to work in Ontario.
QUALIFICATIONS
- College diploma in OTA/PTA required
- Experience with stroke rehabilitation
- Able to work in a professional manner independently as well as with direction required
- Group facilitation skills
- Excellent reporting skills
- Experience and knowledge in working as a member of an interdisciplinary team preferred
- Experience working with individuals who are frail, elderly, and have physical disabilities required

PRINCIPALES RESPONSABILITÉS
Le/la préposé(e) au soutien en réadaptation accompagne les clients dans leurs activités de réadaptation, notamment l'acquisition d'habiletés de vie, afin de favoriser leur autonomie maximale et l'atteinte des objectifs fixés. Il/Elle animera des séances individuelles de réadaptation physique et cognitive et facilitera les groupes d'exercices, sous la supervision d'un(e) ergothérapeute ou physiothérapeute autorisé(e) à exercer en Ontario.
QUALIFICATIONS
Diplôme d'études collégiales en ergothérapie/physiothérapie requis
Expérience en réadaptation post-AVC
Capacité à travailler de façon professionnelle, de façon autonome et en collaboration requise
Aptitudes à l'animation de groupes
Excellentes aptitudes à la rédaction de rapports
Expérience et connaissances du travail au sein d'une équipe interdisciplinaire souhaitables
Expérience auprès de personnes fragiles, âgées ou ayant un handicap physique requise
