Lead PSWs as Home Support Supervisor

Spectrum Health Care (SHC)

Elevate home healthcare as a Home Support Supervisor with Spectrum Health Care in North York, Toronto. Manage and mentor a team of Personal Support Workers for optimal patient outcomes. Spectrum Health Care is looking for a Registered Practical Nurse to lead a community-based team of Personal Support Workers. Your role will center on training, coaching, and overseeing care plans while ensuring high-quality service. Regular travel to patient homes and collaboration with healthcare professionals is essential. Key Responsibilities:

Supervise Personal Support Workers in the community
Assess patient needs and develop tailored care plans
Conduct appraisals and coach for performance improvement
Maintain communication regarding patient concerns
Analyze key performance and satisfaction metrics

Requirements:

Current registration with the CNO
Minimum three years in a supervisory role
Valid driver's license and reliable transportation
Strong interpersonal and communication skills
Familiarity with community nursing practices
