The Role

Supervises operation of the community overnight stay program, ensuring the psychological and social needs of program participants are met to improve their quality of life and that of their care partners in a 24-hour operation.

Provides clinical subject matter expertise for all community programs offered by the Division.

Duties/Responsibilities

  • Ensures service delivery and operations meet regulatory requirements and person-centred service philosophy.
  • Directs and supervises nursing staff for compliance. Works to develop a culture of excellence in clinical practice using a person-centred and emotion-focused care philosophy. Effectively responds to emergent and confidential human resources issues as required.
  • Oversees the overnight stay program and booking calendar to identify clinical risks and case mixing to ensure adequate staff support and skillsets.
  • Provides supportive counseling to participants and their care partners as it relates to service plans, program supports and processes, and community resources.
  • Develops and implements changes in practices and processes to support safe, quality care.
  • Assists and provides coverage for day-to-day human resource functions, including hiring, disciplining, training, and coaching staff to ensure optimal performance.
  • Provides guidance and expertise to regulated staff regarding interpretation and application of leading practice and regulatory standards.
  • Supervises the development and maintenance of participant service plans within Overnight Stay. Updates participant records, information and ensures maintenance of related documentation and information including proper authorization, documentation, storage, and recordkeeping.
  • Participates with staff to gather information through interviews with participants and their care partners. Assesses participants’ mental health status, makes recommendations to physicians for changes, and works with care partners, physicians, care team, and other program supervisors to develop and maintain service plans, including behavioural interventions.
  • Receives and discharges participants into/out of the Overnight Stay program. Occasionally works one-on-one with participants to assist with walking, meals, and reminders regarding medications.
  • Performs administrative tasks (e.g., scheduling, report writing, minute taking, payroll approval, attendance tracking for participants, billing).
  • Completes/participates in quality assurance program activities. Analyzes program data, audit results, and incident reports. Identifies trends and provides feedback and expertise, including recommending process changes to mitigate risks and support continuous improvement in quality care, expert customer service, and compliance with legislative standards.
  • Ensures program medical equipment and supplies are maintained in good working condition.
  • Monitors the program budget. Advises Manager of projected budgetary variances and provides input for budget preparation.
  • Meets daily with staff and team members to discuss participants' physical and emotional wellbeing.

Additional Duties/Responsibilities

  • Holds regular meetings with staff.
  • Performs audits regularly (e.g., medication administration, documentation, education).
  • Consults daily with the Manager regarding program operation, staffing, labour relations issues, crisis intervention, counselling and referrals, and medical crises.
  • Exchanges information with other team members such as arranging scheduling, exchanging participant information, and accessing services through other staff.
  • Provides participant wellbeing and referral information to other organizations offering support/day programs. Provides program information to the public. Performs liaison function with other health care staff and community agencies. Contacts community resources to obtain services on behalf of participants/applicants.
  • Promotes and implements continuous improvements.
  • Participates in community and organization committees to ensure best practice.
  • Backs up the Manager as required.
  • Performs related duties as required.

Knowledge, Skills, and Abilities

  • Knowledge and skills are normally acquired through a Bachelor of Science Degree in Nursing, or equivalent education in the Health Sciences disciplines, plus related experience.
  • Must have current registration with the College of Nurses of Ontario as a Registered Nurse or as a Registered Nurse in the Extended Class. Must be in good standing with the Ontario College of Nurses.
  • Knowledge of dementia and supporting persons with dementia, mental health, and addictions; community program delivery; assessment and supportive counseling; community resources; and pharmacology, plus progressively responsible supervisory/leadership experience in a 24-hour direct service unionized environment.
  • Risk assessment, continuous improvement, and problem-solving skills and ability to implement risk mitigation strategies as needed.
  • Knowledge of and ability to comply with policies, procedures, collective agreements, related legislation (e.g., health and safety, Connecting Care Act, disability, privacy), nursing professional guidelines and standards, Ontario Health, and Ministry guidelines.
  • Leadership skills to supervise, train, develop, motivate, and support staff.
  • Human relations, interpersonal, and communication skills to establish supportive relationships with program participants and their care partners; communicate effectively with staff, other agencies, and the public; present information pertaining to community support services, current issues, and new technologies during activity group discussions and care partner support meetings; and participate as an effective team member.
  • Skill assessing the needs of program participants and their care partners and providing counselling.
  • Ability to write progress notes on program participants and their care partners, monthly reports, and reports/correspondence to community agencies. Ability to read and understand journals, books, newsletters, reports, and correspondence to stay informed.
  • Computer skills with ability to use software such as Microsoft Office.
  • A tuberculosis (TB) test and annual influenza immunization are required.
  • Must provide an acceptable vulnerable sector police check.
  • Ability to participate in an official standby/on-call rotation to respond to legislated or time sensitive demands beyond regularly scheduled work hours.
  • Ability to travel within Waterloo Region.
  • Ability to implement and monitor compliance with the Department’s vision and philosophy for integrated customer service delivery within the division, across divisions, across departments, and within the broader community.
  • Ability to support and demonstrate the Region’s values.
