Keewatinohk Inniniw Minoayawin Inc. is a First Nations-led health transformation organization supporting northern Manitoba First Nations in achieving health sovereignty, improving access to care, addressing gaps in services, and creating racism-free, equitable and culturally safe health systems.

The Minoayawin Mobile Outreach Program (MMOP) provides a bundle of services to unhoused residents of Thompson and surrounding First Nations. The program offers trauma-informed, low- to no-barrier health care grounded in anti-Indigenous racism approaches that honour and give equal validity to Indigenous ways of knowing. MMOP promotes physical, emotional, mental, and spiritual healing in ways that uphold the respect and dignity of unhoused community members living in Thompson, Manitoba, and surrounding First Nation communities.

Reporting to the Program Lead, Minoayawin Mobile Outreach Program, the Social Worker will be a key member of an integrated, multidisciplinary health and mental wellness services team. Working with primary care providers, Elders, Knowledge Keepers, counsellors, peer support workers, and other allied health providers, the Social Worker will support program participants in accessing health, mental wellness, social service, primary care, harm reduction, and cultural supports in Thompson and First Nations communities across northern Manitoba. Daily work will include a combination of outreach, in-clinic, administrative, and preparation activities performed from a central location, mobile outreach settings, and the clinical van in and around Thompson. The role may involve occasional travel within northern communities and work in a northern climate, including winter temperatures, hot summers, and indoor and outdoor wellness van environments.

The successful candidate will help develop, implement, and evaluate wellness plans that support balance across physical, emotional, mental, and spiritual domains in an Indigenous-specific racism-free care environment.

Qualified First Nation candidates will be given preference.

The Social Worker will be responsible to:

·       Connect with MMOP participants in an Indigenous-specific racism-free environment to provide assessment, service planning, treatment, and service coordination that supports healing and wellness for participants, their families, and communities.

·       Conduct psycho-social assessments that include physical, emotional, mental, and spiritual aspects of overall wellness, and work with the participant, family, community, and multidisciplinary team to develop individualized wellness plans.

·       Increase participant access to physical, emotional, mental, spiritual, financial, social, and harm reduction supports.

·       Provide education and service coordination using anti-racist and anti-oppressive social work practice within a multidisciplinary team that may include Elders, Knowledge Keepers, nurse practitioners, peer support workers, physicians, counsellors, and others.

·       Support the provision of harm reduction services, primary care, health promotion, and prevention services for MMOP participants, their families, and communities.

·       Practice in compliance with applicable provincial and federal health privacy legislation, the Child and Family Services Act of Manitoba, the Mental Health Act of Manitoba, and any other legislative mandates that may guide services provided to MMOP participants, families, and communities.

The successful candidate will possess the following qualifications:

·       Bachelor’s degree in Social Work.

·       Current registration and membership in good standing with the Manitoba College of Social Workers.

·       Ability to conduct comprehensive assessments that may include mental wellness status, substance use history, education and employment, social development and functioning, family structure and relationships, activities of daily living, and physical, emotional, mental, and spiritual wellness.

·       Experience providing clinical social work services to individuals, groups, and families in community-based settings or in settings serving First Nations community members and communities facing multiple barriers to health and wellness equity.

·       Knowledge and experience supporting First Nations populations impacted by intergenerational trauma, complex trauma, homelessness, mental wellness challenges, substance use disorders, and inadequate housing.

·       Experience with opioid agonist therapy, Managed Alcohol Programs, and other clinical harm reduction practices is considered an asset.

·       Comprehensive knowledge of health care, allied health, peer support, and people-with-lived-experience disciplines and their roles in holistic participant care.

·       Ability to deal effectively with conflict, manage aggressive behaviour, and use trauma-informed, non-violent crisis intervention and behavioural management techniques.

·       Strong verbal and written communication skills, with the ability to plan, organize, prioritize work, and collaborate respectfully with MMOP team members and partner organizations.

·       Knowledge of First Nations peoples, First Nations cultures, and current critical theory, research, and practice approaches within evidence-based clinical and medical practice is desired.

The following are considered conditions of employment:

·       Ability to travel on occasion as an MMOP team member to First Nations surrounding Thompson, Manitoba, by air, land, and boat.

·       Must maintain membership and remain in good standing with the Manitoba College of Social Workers.

·       Satisfactory criminal record, vulnerable persons, and child abuse registry checks will be required.

·       Successful completion of Personal Health Information Act training will be required.

·       Must be able to physically carry out the duties of the position, including sitting, standing, safely lifting up to 50 lbs., assisting participants with mobility challenges, bending, twisting, and driving for short periods of time.

·       Must be able to work with latex and other materials used in standardized safety equipment.

·       Must be able to work some evenings per month for outings and events based on programming needs.

·       Must possess a valid Class 5 driver’s licence and be eligible to work in Canada.
