This full-time role involves developing therapeutic plans, monitoring drug therapy effectiveness, and collaborating with healthcare teams. You will also deliver comprehensive medication management and ensure patient safety through proactive problem-solving. Key Responsibilities:
- Provide clinical pharmacy services to network patients
- Collaborate with healthcare professionals on therapeutic plans
- Monitor and assess drug therapy and patient responses
- Identify and resolve medication-related issues
- Communicate findings to patients and health practitioners
- Experience in clinical pharmacy practice
- Strong analytical and problem-solving skills
