Job Responsibility:

OVERVIEW

East Toronto Health Partners (ETHP) is the Ontario Health Team (OHT) serving East Toronto. We are a group of more than 100 community, primary care, home care, hospital and social services organizations in East Toronto, as well as patient/client, caregiver and community advisors, working together to better connect local care and services. We work together as an integrated team to make it easier to navigate the healthcare system and transition between healthcare providers and places. Our goal is to build a healthier and more equitable East Toronto - enabling every person and neighbourhood to thrive. In 2023, the Ontario Ministry of Health and Ontario Health named ETHP as one of 12 OHTs to advance rapidly towards maturity. OHTs will be expected to achieve a set of standardized deliverables on an accelerated path including the implementation of two or more integrated clinical pathways (ICPs) over three years. ICPs consist of care teams that deliver proactive, evidence-based care for patients with specific conditions such as congestive heart failure (CHF), diabetes lower limb preservation, and chronic obstructive pulmonary disease (COPD). Through extensive partner engagement and strong clinical leadership, ETHP has co-designed a model of care for ICPs and are now ready to begin design and implementation using an iterative approach.

EDUCATION

• Bachelor's degree in health or business administration required.
• A combination of equivalent education and experience may also be considered.

QUALIFICATIONS

• A minimum of 3-5 year's experience working with people or caregivers in a healthcare setting.
• Project coordination or administration an asset including planning skills, work collaboratively with partners and other team members, and manage deadlines and competing priorities.
• Understanding of local health issues, priorities, and needs
• Knowledge of the East Toronto community and resources
• Awareness of the principles of culturally competent and appropriate care
• Excellent interpersonal, oral and written communication and presentation skills, including ability to plan and implement engagement meetings and resolve issues/concerns
• Experience designing and implementing process improvement projects, preferably in a healthcare setting
• Results oriented with high attention to detail and accuracy;
• Proactive, self-motivated with strong analytical and problem-solving skills;
• Excellent time management skills - can effectively manage multiple priorities and address competing demands
• Excellent capabilities with Microsoft Office suite
