Salary Range: $78,289 to $105,921, plus a comprehensive benefits package, including a defined benefit pension plan

Location: Work from home within the health zones of the Vitalité Health Network (i.e. Edmundston, Campbellton, Bathurst and Acadian Peninsula or Moncton) or based at the NBMS office in Fredericton, NB, subject to operational requirements. This position will focus primarily on supporting physician clinics within the Vitalité Health Network zones. Regular travel throughout the province is required.

Position Overview

The Practice Facilitator works directly with physicians, nurse practitioners, and clinic teams across New Brunswick to support practice improvement initiatives that enhance patient care, team effectiveness, clinic operations, and access to care. This dynamic role combines facilitation, coaching, quality improvement, change management, and stakeholder engagement to help clinics identify opportunities, implement sustainable improvements, and strengthen team-based care delivery.

As a trusted advisor and facilitator, you will help physicians and clinic teams identify opportunities for improvement, implement meaningful change, and build sustainable practices that support better outcomes for patients and providers.

Key Responsibilities

Practice Facilitation and Improvement

• support physician practices in identifying and implementing quality improvement opportunities
• facilitate workflow reviews, assessments, and improvement planning
• assist clinics in strengthening team-based care and operational effectiveness
• support implementation of evidence-informed tools, processes, and best practices

Coaching and Change Management

• provide coaching and facilitation to clinic teams
• support clinics in implementing sustainable change initiatives
• connect teams with provincial resources and improvement opportunities
• support teams in measuring outcomes and evaluating improvement efforts

Health Partner Collaboration

• work with physicians, healthcare partners, government, and internal stakeholders
• support provincial practice support projects and initiatives
• contribute to provincial initiatives that strengthen access, quality, and continuity of care

Qualifications

Relevant experience may come from a variety of educational and professional backgrounds. Candidates who do not meet every listed qualification are still encouraged to apply if they believe their experience would support success in this role.

Education

• post-secondary degree or diploma in healthcare administration, business administration, health sciences, nursing, education, public administration, social sciences, or a related field
• equivalent combinations of education, training, certifications, and experience may be considered

Experience

• experience working within healthcare, primary care, or a related sector
• experience in quality improvement, facilitation, coaching, project management, or change management
• experience supporting organizational improvement initiatives and stakeholder engagement activities
• experience leading workshops, meetings, or group facilitation activities
• experience working with physicians, healthcare teams, or healthcare organizations is considered an asset

Knowledge, Skills, and Abilities

• understanding of quality improvement methodologies and change management principles
• knowledge of primary care environments, team-based care models, and healthcare operations
• strong communication, facilitation, and relationship-building skills
• ability to support diverse stakeholders and build consensus
• strong organizational, planning, and time management skills
• ability to analyze information and develop practical recommendations
• ability to work independently and collaboratively
• proficiency with Microsoft 365 applications and workplace technology
• strong problem-solving and critical-thinking abilities

Language Requirements:

• Written and spoken competence in English and French is required. Language proficiency will be assessed during the recruitment process.

Position Location and Travel

This position may be based remotely within New Brunswick in one of the Vitalité Health Network Health Regionals or from the NBMS office in Fredericton, subject to operational requirements. The role provides the opportunity to work directly with physician practices and healthcare teams across the province, supporting meaningful improvements in primary care delivery. Regular travel throughout New Brunswick is required, with a focus on Vitalité Health Network zones. Candidates must possess a valid driver's licence and have access to reliable transportation. Occasional work outside normal business hours may be required to accommodate physician schedules, clinic meetings, workshops, and program requirements.
