Relief Personal Support Worker, Long Term Care

Job Information

Personal Support Workers (PSWs) provide individualized care to residents in Northwest Territories (NWT) Long Term Care (LTC) homes and Extended Care Units (ECUs). Their primary responsibility is to support residents' independence, assist with daily living activities, and ensure care respects cultural diversity, personal preferences, and individual dignity.

The goal of the position is to create a home-like environment that respects each resident's unique needs, choices, and abilities while promoting independence and encouraging continuous learning and professional development.

The Northwest Territories Health and Social Services Authority (NTHSSA) is responsible for delivering most health and social services across the NWT, serving approximately 43,000 residents across 1.2 million square kilometers. Services include primary, secondary, and tertiary healthcare, as well as social services such as family support, mental health, addictions, and community programs.

Within LTC homes and ECUs, PSWs are key members of the care team. They work under the direction of and report to Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) to provide quality resident-centered care.

Key Responsibilities:

• Provide individualized care to residents in accordance with the policies, standards, and objectives of the NTHSSA and the principles and philosophy of Supportive Pathways to ensure the continued health, safety, comfort, and independence of residents.
• Ensure residents¿ continued health and well-being by providing timely and "person-focused" activities of daily living (ADL) and assistance with Instrumental Activities of daily living (IADL) with little to no restrictions.
• As a member of the care team, the incumbent will actively contribute to the mental, spiritual, psychological, and social well-being of residents.
• Advocate for practice environments that have the organization and resource allocations necessary for safe, competent, and ethical care.
• Evaluates, documents and reports findings.

Key Qualifications: 

• The successful completion of a Personal Support Worker or equivalent Certification Program; or if no certification, must have at least 1 year of work experience with a minimum of 1000 hours providing care in a long term care or home and community care setting and be able to complete the PSW course within 5 years of hire.
