Remote Nurse Case Manager in Canada

Elevate patient care as a Remote Nurse Case Manager with Sentrex Health Solutions in Canada. As a Nurse Case Manager (NCM) at Sentrex Health Solutions, you'll be a vital resource within the Patient Programs department. This full-time remote role involves engaging with patients, healthcare providers, and insurance companies to manage complex cases and provide valuable support. Your primary responsibility will be facilitating access to pharmaceutical care and navigating reimbursement processes effectively. Key Responsibilities:

Perform patient and healthcare provider registrations
Conduct data entry for program services
Offer support through various communication platforms
Assist with medication education and disease management
Collaborate with reimbursement specialists for funding

Requirements:

RN or RPN in good standing
Minimum 2 years clinical experience
Knowledge in Patient Support Programs
Experience in Neurology is a strong asset
Proficient in case management protocols
