Community-Based Nurse Practitioner Position

Key Responsibilities:
- Deliver comprehensive care services to diverse populations
- Conduct assessments and maintain clinical records
- Facilitate health education and counseling sessions
- Evaluate and implement disease prevention programs
- Collaborate effectively in team-based initiatives
- Accredited Primary Care Nurse Practitioner program
- Master’s degree in nursing and NP certification
- At least two years in community health
- Familiarity with community services and health legislation
- Valid Ontario Driver’s License
