Job Summary

This position works within the Nurse Practitioner's (NP) scope of practice in the category of practice of Family/All Ages. This is an advanced practice role providing professional, analytical and an autonomous comprehensive range of health services to clients and patients including but not limited to health assessments and diagnosis of diseases, disorders, injuries and conditions, treating and managing acute and chronic illnesses and identifying health risks and needs within a holistic model. In this role the nurse practitioner performs all activities utilizing a harm reduction approach within the primary health care framework.

The PHC Team mandate is to provide low barrier access to harm reduction focused, primary health care services for individuals who are homeless or precariously housed. Many of these individuals are navigating complex mental health and/or substance use conditions and often experience marginalization because of their circumstances; many have trouble accessing traditional health care services.

The NP will work to full scope of practice providing care to the vulnerable population through a walk-in clinic and/or outreach to various locations (Mobile Clinics, community partner locations etc.) They will also be involved in the Managed Alcohol Program (MAP) and a Palliative approach Care for vulnerable populations, in collaboration with other providers and team members.

The nurse practitioner is a member of the Downtown Healthcare Collaborative and harm reduction teams of health professionals including, but not limited to, registered nurses, pharmacist, occupational therapist, community outreach workers, social worker, peer support workers and others, working across programs and services to provide the best possible care. Health services in this context are provided in a variety of ways including street reach and mobile clinics, home visits, PHC walk-in clinics, community partnerships, and acute care partnerships.

The nurse practitioner, as a member of the team, provides for the presenting needs of the client and includes all routine primary health care and treatment as well as Hepatitis C, HIV, latent TB, STBBI testing and treatment, mental health single session counselling, client education, and is a satellite site for provision of SWAP supplies. All services are intended to reach the most vulnerable, underserved individuals in our community.

Constantly moves, lifts, or operates equipment/machinery up to 10 lbs. and regularly is required to lift/move equipment, books, supplies, and sometimes children up to 25 lbs. (may need to be deleted or added depending on the position). Occasionally, may have to reposition a patient over 50 lbs.

Physical effort also includes regular sitting to conduct patient histories, performing work on the computer and when assessing patients, regularly stands or walks.

When performing procedures, regularly uses fine finger/precision work and hand tools (i.e. medical instruments, computer mouse, stethoscope, etc.) requiring accurate control and steadiness.

Visual concentration is required when performing physical assessment and procedures, completing charts/reports, observing patients, reading equipment/monitors, and completing work-related correspondence on the computer.

Job Qualifications

Completion of an approved/recognized Nurse Practitioner program is required.

Current registration with the College of Registered Nurses of NL and Licensed (or eligible for licensure) as a Nurse Practitioner (Family/All Ages) with the CRNNL is required.

A minimum of five years clinical experience within the last seven in Primary Health Care is required.

Current BCLS Certification is required.

Possessed strong communication, problem solving, decision-making, and teaching skills.

Demonstrated skills in computer and database management skills

A valid Newfoundland and Labrador Class 5 Driver's license and use of private vehicle is required.

A satisfactory record of work performance and attendance is required.
