Ontario Health atHome is seeking a Bilingual Information & Referral Representative to provide accessible, patient-centered information and referrals across Champlain region. You will be the first point of contact handling calls in the caller's language of choice and connecting clients to health and community services.
Responsibilities include responding to inquiries, linking to care coordinators, promoting Champlainhealthline.ca, and maintaining confidentiality.
