As the Registered Practical Nurse , you will be part of an interdisciplinary team, who is expected to ensure the safety and security of the resident environment, to practice in accordance with College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services. The RPN will be responsible for working with individuals from diverse backgrounds, contributing to Program development, participating in clinical supervision; and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. The RPN is directly responsible for developing and articulating strategies for the management of recovery and psychosocial rehabilitation using information collected through assessments and collaboration with the PSWs. Additionally, you will liaise with community agencies, other practitioners, and key partners to coordinate care for clients.

LOFT was chosen to be the Homecare and Community Services provider for CAMH's new Hospital to Home (H2H) program. The standard Service Period for a H2H Program is sixteen (16) weeks in duration, with one Service Bundle to be assigned to the Patient and delivered over the course of this defined timeframe, with Services provided in accordance with the Patient Transitional Care Plan. The H2H program is designed to support adults 55+ facing complex mental health challenges, addiction, dementia and responsive behaviours along with physical health and care needs. This program will focus on individuals lacking access to appropriate supports while living in the community and other social determinants of health. The H2H LOFT Service Team will be working closely with CAMH partners as part of an integrated care model and ensures contact with referred individuals in the community within 24 hours. As part of this mobile team, staff will be travelling across the GTA to meet and work in a variety of environments to support individuals where they are at including in their homes.

The Path Home (TPH) is a 48-unit ALC transitional housing program designed to provide supportive care to individuals living with mental health challenges. Our program aims to create a nurturing environment where clients can transition from acute care settings to a more stable living arrangement. Our program operates in partnership with the Centre for Addiction and Mental Health (CAMH) and the University of Health Network (UHN) hospitals, exclusively accepting referrals from these organizations. Our dedicated nursing staff offers medical monitoring, wound care, and medication management, while clients also have access to primary care services and case management services. Life skills training and community engagement activities empower our clients to live independently and experience meaningful connections. Our program aims to holistically address physical, mental, and emotional needs, fostering a smoother transition, collaborative care, and empowerment for a successful reintegration into the community

Provides direct nursing care with medications as defined by the College of Nursing Practice including

transcribing medical orders as given by physicians in writing or by telephone

follows all policy and procedures as outlined by the Pharmacy.

Identify changes in appearance/behaviours, reporting physical and mental health concerns, unusual or exceptional occurrences and behaviour.

Participate in service planning and development of personal goals for clients to transition to their permanent discharge location;

Provide support with behaviours arising from cognitive issues, intervening in conflicts between residents, and contacting appropriate emergency services in crisis situations;

Ensure proper care is rendered in a safe and clean environment;

Liaise with physicians, pharmacists and other health care providers regarding client care;

Provide advice, coaching and support to staff regarding medical/healthcare questions and concerns;

Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles;

Maintain strict confidentiality and follow all other required policies and procedures;

Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on LOFT Community Services;

Participate in on call rotation regularly and as required;

Follows standard infection control practices and enhanced precautions as instructed;

attend staff/team meetings, client care conferences, other meetings etc. Serve as back up to Personal Support Workers, as required;

Attend and actively participate in all required training;

Currently registered with the College of Nurses of Ontario;

Graduate of a recognized program for Registered Practical Nurse

Medication Administration Certification or completion of an RPN education preparation program that includes medication administration

Experience with vulnerable older adults and seniors with serious mental illness, cognitive, addiction and responsive behavioural challenges who have been longer term hospital patients;

First Aid and CPR certification;

Knowledge of social services and community resources and supports for adults and seniors;

Criminal Records Check and Vulnerable Sector Screening

An understanding of recovery and psychosocial rehabilitation principles and the therapeutic relationship;

Counselling, responding and assessment skills;

Good oral and written communication skills and computer literacy, including MS Word and Excel;

Ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members;

Occasional evening and weekend hours may be required.

A car is not a requirement of the position however driving for prolonged periods of time with frequent stops is an essential part of the job. You may need to walk on rough terrain from time-to-time, for example uneven ground, parks, or ravines while providing outreach to community members sleeping outside.
