Diététicien Nutrition (H/F)

As a Clinical Dietitian you will be an integral part of a collaborative team focused on delivering seamless services and programs to our clients and customers. Conducts comprehensive nutrition assessments for patients.
Develops and implements individualized nutrition care plans
Provides counselling and education and in-services to patients using trauma-informed and patient centered approach
Documents as per HHC and college standards including assessments interventions and outcomes in patient medical record
Bachelors degree in Human Nutrition and Dietetics from an accredited post-secondary institution
- Valid registration with College of Dietitians of Ontario is required member of Dietitians of Canada preferred
- 5 years previous experience in Older Adult and Eating Disorders
- Experience with mental health and addictions populations
- Commitment to working within Homewoods Health and Safety Standards
- Practice in accordance within the ethical guidelines of the Dietetics profession
