Current Shifts : The primary role of the Nurse Practitioner (NP) is to provide high quality, safe, effective and evidence-based care in partnership with patients and families, in accordance with the College of Nurses of Ontario Nurse Practitioner Practice Standards and GGH’s mission, vision, values and policies. The NP uses advanced nursing knowledge clinical practice expertise and provides leadership to promote optimal patient health, program development, process improvements and the enhancement of professional standards. Nurse Practitioner (Registered Nurse in the Extended Class) with certificate of registration with the College of Nurses of Ontario with license to prescribe controlled substances

  • Master’s degree in Nursing or equivalent preferred
  • Completion of the Prescription and Management of Controlled Drugs and Substances Course
  • Experience in development and implementation of Medical Directives and Policies and Procedures
  • Strong assessment skills of the complex medically acute adult patient
  • Advanced knowledge and skill working with various types of dementia, delirium, mental health issues, addictions and other neurological conditions in older adults.
  • Demonstrated specialized skills in geriatric medicine health assessment, care planning and patient outcomes
  • Advanced knowledge of psychotropic medications in symptom management, monitoring/evaluating effectiveness and side effects
  • Training in P.I.Montessori Methods in Dementia, Validation Communication
  • Extensive knowledge of community partnerships and geriatric resources in Guelph and Wellington County
  • Ability to function as a content expert in geriatric care and act as an expert resource to the inter-professional team.
  • Demonstrated commitment to best practice, nursing standards and life-long learning
  • Demonstrated commitment to excellent customer service when interacting with patients, families, colleagues and physicians
  • Demonstrated leadership in committee work, education, quality improvement, policy & procedure development, and knowledge translation initiatives
