Working as an integral member of the inter-professional team, you will play a critical role in providing social work services to patients and their families within the organization, in the acute care setting, with the exception of in-patient mental health. The successful candidate will demonstrate exceptional leadership skills, a high level of professional integrity and accountability and play a key role in system development, as well as possess a strong level of understanding of care across the continuum. Experience in short-term supportive counseling with patients and families dealing with difficult decisions and end of life concerns would be a valuable asset.

Short-term, goal oriented support and counseling for patients and families dealing with grief, chronic life altering issues as well as end of life concerns

• Working with the health care team, patient and family to develop patient care plans
• Cross coverage for other Social Workers within the hospital
• Consultation with community health care professionals
• Maintaining up dated knowledge of community resources
• Master of Social Work
• Registration with the Ontario College of Certified Social Workers and Social Services Workers
• Recent related previous social work experience in a hospital environment
• Demonstrated good attendance and performance records with the ability to maintain these same standards
• by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required
