Physiotherapist, Physiotherapy & Occupational Therapy

Position is for a 0.5 FTE on Community-Based Stroke Outreach and 0.4 FTE on 3N Physical Rehab.
Reporting to the Manager – Regional Rehabilitative Care Program (RRCP), the Physiotherapist (PT) working with the Community-based Stroke Outreach Program will provide therapy (in person and virtually) within a regional outreach service specific to clients with stroke and those with chronic diseases. The Community-based Stroke Outreach Program is a service designed to:
Provide access to community-based stroke rehabilitation for the region East of Thunder Bay (ie. Nipigon, Terrace Bay, Manitouwadge, etc) and to remote, fly-in First Nations.
Services will be both in-person in the community the client resides as well as virtual. In-person will be a combination of outpatient, partnering with the local hospital/First Nation Home and Community Care, and providing services in the client’s home.

The Physiotherapist will work within a regional team, including: Occupational Therapy and a Stroke Navigator. The team will also have support from the tele-speech Speech Language Pathologist as required.

Physiotherapy is a regulated health profession that enhances quality of life by managing and preventing many physical problems caused by illness, chronic conditions, sport and work related injury, aging, and inactivity. The primary objective of physiotherapy is to promote wellness, mobility and independent function.
The PT is responsible for triaging referrals, assessing clients, establishing a clinical impression/diagnosis, and providing recommendations and treatment to address client goals and enable clients to maximize their level of independence in function and mobility. The PT will work as part of an interprofessional team and acts as a consultant to other disciplines and outside agencies.
The 0.4 FTE assignment to Special Rehabilitation is an exciting opportunity to work with clients who have experienced a stroke , acquired brain injury , or spinal cord injury . You will be part of a supportive, multidisciplinary team committed to delivering high-quality, people-centred care.
Our Special Rehabilitation Unit provides a rich environment for clinicians looking to develop and refine their skills in neurological rehabilitation .
Provides high-quality, people-centered physiotherapy care within the scope of practice for Physiotherapists in Ontario.
Attends interprofessional team meetings and actively contributes to collaborative care planning to support both optimal rehabilitation outcomes and effective discharge planning.
Supervises and supports the work of Rehabilitation Assistants, ensuring safe and effective implementation of therapy plans.

A degree or equivalent from an accredited Physiotherapy program
Current unrestricted registration with the College of Physiotherapists of Ontario (CPO)

Ability to manage a clinical practice in a health care setting
Ability to complete analysis, using clinical knowledge, skill and judgement to synthesize assessment information
Ability to assess and treat acute and chronic neurological, orthopaedic, geriatric and cardio-respiratory conditions
Ability to communicate effectively and concisely, both orally and in writing
Strong project/time management, analytical/critical thinking, problem-solving, and decision-making skills
Asset:
Experience with virtual care technology (i.e. Ontario Telemedicine Network (OTN), teams, etc) is an asset
Assistive Devices Program (ADP) authorization is an asset
Membership in Canadian Physiotherapy Association (CPA) and Ontario Physiotherapy Association (OPA) are strongly encouraged assets

Will be required to travel to various work locations throughout Northwestern Ontario and provide own transportation
