Director of Rehabilitation Services

JOB DESCRIPTION


Position Summary : The Director of Rehabilitation Services is responsible for oversight of the provision of the Medicaid billable rehabilitation services within the agency. These services include Health Home Care Management, CORE (Community Oriented Recovery and Empowerment Services, the OASAS CARES program and the OPWDD Waiver Day Services. Responsible for the supervision, implementation, monitoring and marketing of Gateway's Rehabilitation services.

Essential Responsibilities :

  • Plan, develop and implement Gateway's Rehabilitation services for individuals recovering from psychiatric disabilities.
  • Develop and implement the CORE services, including policies, procedures, program budget and structure.
  • Plan and implement for program growth in all rehabilitation services, including increasing referrals and enrollment numbers for programs.
  • Coordinate referrals, conduct intakes and assign the cases to designated staff in Rehabilitation Services.
  • Plan, develop and implement funding for Rehabilitation Services.
  • supervise and monitor services funded by Medicaid, Managed Care Organizations, Office of Mental Health and Department of Health, OPWDD (Office of People with Developmental Disabilities) insuring compliance with all agency, county, state and federal regulations and procedures.
  • Develop and implement policies and procedures for services and for training direct care staff.
  • Function as a member of the Lead Health Home and Care Management team in Ulster County.
  • Participate in county-wide care management trainings, case reviews, and utilization reviews.
  • Participate in treatment team meetings, upon individual and practitioner request, for the purpose of facilitating Rehabilitation Services.
  • Provide coverage for services, as needed.
  • Provide linkage and referral facilitation for services, resources, and tools on behalf of Rehabilitation service recipients to support them in achieving their goals.
  • Prepare and maintain records of individual progress and services performed, reporting changes in the electronic health record.
  • Abide by HIPAA laws, regulations, and related agency's policies and procedures.
  • Provide outreach and presentations to mental health practitioners regarding rehabilitation services for Gateway.
  • Track utilization of services and referrals to Rehabilitation services and monitor results of those referrals.
  • Maintain communication and relationships with individuals, outside agencies, referral sources, and other service providers.
  • Ensure the achievement of program objectives and expectations.
  • Represent the agency's Rehabilitation services on community committees.
  • Keep abreast of trends, changes, and new developments in the field through professional affiliations, community involvement, conferences, and trainings.
  • Monitor program quality and documentation compliance through regular review of case records. Make recommendations and implement changes for improvement in program quality.
  • Evaluate program results and achievement of productivity, revenue generation and strategic goals. Collect and review monthly statistical reports; submit quarterly reports to the Vice President of Vocational & Therapeutic Services.
  • Complete and review billing information for accuracy; submit billings to accounting and regulatory agencies.
  • Assist in the development of program budgets and management of cost centers.
  • Serve as a member of the Direct Services Team, Corporate Compliance Committee and Incident Review committees.
  • Participate in training programs for professional growth as scheduled and approved.
  • Provide a supportive environment and function as a function model.
  • Maintain a work environment in a clean, organized and safe manner.
  • Assure direct reports supervise and evaluate employee health and safety performance and staff knows what to do in the event of an emergency. Assure staff is competent in the recognition and management of potentially dangerous situations. Assure staff reports near misses and incidents as required by regulation and safety standards of practice. Investigate serious incidents for their root causes and uses the events as opportunities for systems improvement and staff development.
  • Responsible for knowledge and adhering to Gateway Hudson Valley's Corporate Compliance Policy and Standards of Conduct. Will keep abreast of all pertinent federal, state and Gateway Hudson Valley regulations, laws and policies as they presently exist and as they change or are modified. Insure that staff is knowledgeable of all applicable regulations, laws and policies. Ensure all accidents are promptly reported to Human Resources. Qualifications
  • Minimum Qualifications and Required expertise: Bachelor's Degree required, five years' practice in the field, with two years' and practice in a supervising rehabilitation services.
  • Preferred Qualifications required Master's degree in Human Services or related field preferred, with one year practice providing direct service related to rehabilitation and/or the care or treatment of persons with a designated mental illness diagnosis, Substance Use Disorder, and practice in supervising a rehabilitation service.

Must have a valid NYS Driver's License and maintain a driving record, which meets the insurable criteria of the Agency's automobile insurance carrier. Must have a favorable result from the federal/state exclusions background check as well as a background check facilitated through fingerprinting and mandated by NYS law Chapter 575.
