Social Worker, Primary Care (full Time)

WellFort is seeking to hire Fulltime Permanent- 1.0 FTE Social Worker!

WellFort Community Health Services is a non-profit values-based organization based in Peel Region. We provide a wide variety of comprehensive care including primary care dental mental health and chronic disease management with a focus on illness prevention health promotion and community development. We are currently recruiting for a Social Worker to provide clinical counselling and case management as a part of our Bloom team. Bloom Clinic is one of several provincially funded Hepatitis C outreach teams providing testing treatment and care to people who use drugs and people who are street involved across Peel Region. The successful candidate will have a strong foundation in harm reduction and work from an anti-racist anti-oppressive and trauma informed framework.

Provide counselling and case management to individuals living with at risk of and affected by HIV and HCV across Peel Region

• Provide community-based education sessions on various issues including HIV HCV Harm Reduction Mental Health and more.
• Screen/assess for self-harm/risk factors and create safety plans with clients as needed.
• Engage in individual and systemic advocacy related to needs articulated by community members
• Build and maintain strong relationships with community partners.
• Actively participate internal meetings as well as external community tables and committees

• Member in good standing with Ontario College of Social Workers and Social Service Workers and licensed to practice the controlled act of psychotherapy.
• Experience working with people experiencing homelessness people who use drugs people who are incarcerated or otherwise involved in the criminal justice system youth and Indigenous community members
• Lived/living experience with any of the populations served by Bloom Clinic is an asset
• Experience working with communities and individuals who face barriers accessing tradition health care institutions such as poverty discrimination mental health and substance use issues.
• An understanding of and critical analysis of the determinants of health and social factors such as power dynamics that affect individual organizational and community health.
• Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level.
• Experience working in a value based non-profit or social services organization.
• Demonstrated proficiency in use of computers and various computer software applications such as Electronic Medical Record (EMR).
• Ability to speak languages relevant to the residents of the organizations community is an asset.
