The Pharmacist is responsible for meeting the medication related needs of patients which will include resolving and preventing drug related problems. The principle responsibilities include working with patients and the healthcare team to provide direct patient care, participating in patient education, facilitating medication reconciliation activities, teaching of students, providing education to nursing and medical staff, and supporting the drug distribution system.
Current registration in good standing with the Ontario College of Pharmacists in Part A of the register;
Practical knowledge of pharmacology, current best practice clinical guidelines, and ability to interpret lab values
Demonstrated ability to research and provide drug information effectively to patients and other members of the multidisciplinary team;
Previous hospital experience desired;
