Registered Practical Nurse (RPN) - Nursing Home

RPN Location Hamilton, ON : About Us BeHome is focused on delivering high-quality, compassionate care tailored to meet the unique needs of our patients. If you're an experienced and dedicated RPN seeking a flexible, casual role, we invite you to join our team! We are hiring on a casual basis, looking for coverage for a client in Simcoe for 2 days a week from 9-5, with the possibility of including weekends. Deliver direct patient care in diverse healthcare settings, including acute care, assisted living, and medical offices.
Administer medications and treatments as prescribed by healthcare providers.
Monitor and assess patient conditions, focusing on pain management and symptom control.
Collaborate with interdisciplinary teams to create and implement individualized care plans.
Assist in case management and coordinate care for patients with complex medical needs.
Accurately document patient information in electronic health record systems such as Cerner.
Educate and support patients and their families regarding healthcare needs and care plans.
Minimum of 1 year of experience as a Registered Practical Nurse (RPN).
Experience in acute care, dementia care, hospice care, or other specialized areas is an asset.
Familiarity with medical office procedures and healthcare documentation.
Demonstrated ability to provide compassionate, patient-centered care.
Exceptional communication skills for effective interaction with patients, families, and healthcare teams.
Current registration with the College of Nurses of Ontario (CNO).
Completion of an RPN certificate program.
