Salary Range: The salary range for this position is $97.29 - $105.57 per visit.

Be the connection patients need beyond the hospital. As a Social Worker in our Integrated Care Program , you’ll support smooth transitions home by addressing psychosocial needs, connecting patients to community resources, and collaborating with an interdisciplinary team to improve outcomes and reduce readmissions.

You have a Bachelor of Social Work or a Master of Social Work
- You are a registered member in good standing with the Ontario College of Social Workers and Social Service Workers
- 1 to 2 years of community experience is an asset
- You have ability to travel within the Hawkesbury community as part of the CBI Health Home Health Services team
- Your bilingual language skills in English and French are a MUST have
