Rehabilitation Support Workers Job Description Template
Our company is looking for a Rehabilitation Support Workers to join our team.
Responsibilities:
- Communicate to regulated healthcare professionals regarding client’s progress and intervention strategies;
- Liaise with all members of the treatment team to ensure effective service provision and attend team meetings as scheduled;
- Provide input into treatment objectives and client’s programming when requested;
- Conduct self in professional and courteous manner at all times;
- Communicate with Manager on a regular basis, seeking guidance when necessary;
- Review all documentation provided by members of the treatment team;
- Prepare and send detailed progress notes to Manager for each client on a weekly basis, as specified;
- Exercise good personal time management and consistent punctuality;
- Effectively and professionally implement treatment programs under the direction of the rehabilitation team;
- Meet with clients as specified by the treatment team and as set out in the treatment plan;
- Prepare and send progress reports to Manager for each client on a regular basis, or as specified;
- Participate in ongoing training and professional development in-services;
- Treat every client with dignity and respect.
Requirements:
- Maintain up-to-date criminal reference checks including vulnerable sector screening;
- Updated CPR/First Aid Certificate;
- Maintain certification in Non-Violent Crisis Prevention and Intervention;
- Valid driver’s license and clear driver’s abstract.
