Our company is looking for a Rehabilitation Support Workers to join our team.

Responsibilities:

  • Communicate to regulated healthcare professionals regarding client’s progress and intervention strategies;
  • Liaise with all members of the treatment team to ensure effective service provision and attend team meetings as scheduled;
  • Provide input into treatment objectives and client’s programming when requested;
  • Conduct self in professional and courteous manner at all times;
  • Communicate with Manager on a regular basis, seeking guidance when necessary;
  • Review all documentation provided by members of the treatment team;
  • Prepare and send detailed progress notes to Manager for each client on a weekly basis, as specified;
  • Exercise good personal time management and consistent punctuality;
  • Effectively and professionally implement treatment programs under the direction of the rehabilitation team;
  • Meet with clients as specified by the treatment team and as set out in the treatment plan;
  • Prepare and send progress reports to Manager for each client on a regular basis, or as specified;
  • Participate in ongoing training and professional development in-services;
  • Treat every client with dignity and respect.

Requirements:

  • Maintain up-to-date criminal reference checks including vulnerable sector screening;
  • Updated CPR/First Aid Certificate;
  • Maintain certification in Non-Violent Crisis Prevention and Intervention;
  • Valid driver’s license and clear driver’s abstract.
