Expected Start Date: September 09, 2026 Union: SUN Facility: Nipawin Hospital City/Town: Nipawin Department: Clinical Integration Chronic Disease Management Type: Full-time regular FTE: 1 Number of Hours per Rotation: 112 hours per 3 weeks Relief: No Float: No Hours of Work: Standard Hours Salary or Pay Band: Pay Band Nurse B $42.040 to $52.440 Travel Required: Yes Job : Assisting with developing, coordinating and conducting inpatient and outpatient programs and clinics; using established criteria to assess and plan each client's self-management care needs, providing intervention ensuring timely access to care; providing efficient and effective individual self-management education/counseling for patients/families/caretakers in all aspect of management care and treatment. This includes liaising with families, physicians, dietitians, staff nurses and community agencies. Human Resources Exemption: No Education- Bachelor of Science in Nursing (BScN)

- Basic Life Support Certificate (BLS)
