Field Case Manager - 12 Months Contract

Cencora seeks a Field Case Manager for a 12-month remote contract role based in Ontario, Canada. The position involves coordinating patient care journeys, supporting healthcare providers, managing reimbursement processes, and maintaining clinical documentation while working from a home office with occasional in-person visits.

Responsibilities

• Serve as primary contact and build relationships with physicians, patients, and program stakeholders
• Educate healthcare providers on program services, reimbursement processes, and enrollment requirements
• Review patient charts and manage documentation, authorizations, and prescription renewals
• Oversee special authorization submissions and provide reimbursement guidance to healthcare teams
• Attend continuous healthcare education events with providers outside regular business hours

Requirements

• Registered Nurse (RN), BSc in Pharmacy, or equivalent science degree
• Knowledge of public or private third-party reimbursement systems
• Valid, unrestricted driver's license and personal vehicle
• Bilingual capability in English and French for Quebec and New Brunswick
• Strong computer proficiency including Microsoft Office
• Previous case management experience preferred
