Bilingual Care Navigator (Home Health)

Ontario Health atHome is hiring a bilingual Care Coordinator to assess patient needs, determine eligibility, and connect patients to community services. You will lead home visits and coordinate with caregivers and clinicians in congregate settings and aging in place environments.

The role emphasizes collaboration, care planning, and patient-centred service delivery, with regular reporting to physicians and care teams. Office, community, and facility-based work options are available.
