ABOUT THE ROLE
We are seeking compassionate healthcare professionals to join our dedicated interprofessional team, providing specialized care with Palliative and Transitional Care Services.
Our palliative care program focuses on exceptional pain and symptom management, as well as end-of-life care, ensuring the best possible quality of life for clients with advanced diseases and complex needs. Simultaneously, our Transitional Care program provides comprehensive, personalized care in a centralized setting for clients awaiting Long Term Care placement, addressing their unique health and wellness needs.
Working alongside Palliative Physicians, Nurses, Therapists, Social Workers, and other specialists, you will contribute to developing and implementing wholistic care plans tailored to each adult client’s circumstances. The Registered Nurse (RN) is responsible for assessing clients, developing a plan of care, implementing treatments, and evaluating the outcomes, in consultation with the client and/or other health professionals, in accordance with the Standards of Practice for the College of Nurses of Ontario (CNO). The RN works as part of an interdisciplinary team.

WHAT YOU BRING
Education/Experience:
- Certificate of Registration with the Ontario College of Nurses (CNO)
- Basic Life Support Certificate (BLS)
- Education and experience in a hospice/palliative care setting
Skills/Abilities:
- Leadership experience preferred
- Excellent physical assessment skills
- Recent experience in the administration of medications
- Ability to work independently and as part of a team
- Demonstrated ability to work within a palliative care setting
- Demonstrates strong communication skills, including clear and compassionate verbal, written, and interpersonal abilities essential for professional collaboration
- Evidence of continuing professional development experience
- Demonstrated regular attendance at work
Asset:
- Canadian Nurses Association Hospice Certification
Conditions of Employment:
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
